Termination of pregnancy services are inspected under a specific CQC strand. The five key questions apply, with heavier weight on records that demonstrate compliance with the Abortion Act 1967 and its supporting regulations: two-doctor authorisation under HSA1 forms, the HSA4 notification of termination to the Chief Medical Officer within the statutory timeframe, gestational-age documentation, consent records (including for under-16s and those lacking capacity), counselling availability records, and the safeguarding evidence around coercion and abuse disclosure during consultation.

TOP providers satisfy CQC for regulated activity, the Department of Health and Social Care via the Chief Medical Officer notification path, and (for NHS-contracted providers) the contracting body. The record-keeping bar is unusually high for the sector because the statutory framework around abortion creates very specific documentation duties that other healthcare sectors do not have.
